In practice, that means a validated system with personal user accounts and role-based permissions, an automatically generated audit trail that no one can edit (who, what, when, why, old and new value), electronic signatures showing name, timestamp and meaning that are permanently linked to the document, and controlled versioning with approval workflows. The governing frameworks are 21 CFR Part 11, EU GMP Annex 11 (currently under revision), ICH E6(R3) and the ALCOA+ principles. Responsibility for validation and data integrity always stays with the regulated company, even when the supplier provides validation documentation. Which rules apply in practice depends on the documents the system holds and the markets the company serves.

21 CFR Part 11: when is a document an “electronic record”?

Under §11.1(b), 21 CFR Part 11 covers electronic records created, modified, maintained, archived or transmitted under an FDA predicate rule, plus records submitted to FDA electronically. An SOP, protocol or monitoring report held in an eDMS rather than on paper is therefore in scope. The core controls sit in §11.10: validation (a), accurate and complete copies (b), protection of records throughout retention (c), limited access (d), a computer-generated, time-stamped audit trail (e), operational, authority and device checks (f to h), training (i), written accountability policies (j) and control of system documentation (k).

For signatures, §11.50 requires the printed name, date and time, and meaning (such as review or approval); §11.70 requires signatures to be linked to their records so they cannot be excised, copied or transferred. §11.100 to §11.300 cover uniqueness, written certification to FDA, two distinct identification components (for example user ID and password) and password controls. FDA’s 2003 “Scope and Application” guidance announced enforcement discretion for validation, audit trails, copies, retention and legacy systems, but not for access controls or signatures; predicate rules continue to apply in full.

The final FDA guidance “Electronic Systems, Electronic Records, and Electronic Signatures in Clinical Investigations: Questions and Answers” (October 2024, Revision 1) applies this to clinical research: records may be kept in cloud services provided authenticity, integrity and metadata are preserved (Q5); validation is risk-based and may draw on an IT provider’s documentation (Q7); sponsors should hold a data-flow diagram (Q8); and FDA does not pre-assess systems but evaluates them at inspection (Q16). Regulated entities remain responsible for Part 11 compliance even when IT services are outsourced. For more depth, see our 21 CFR Part 11 electronic signatures guide.

EU GMP Annex 11: the current text and the revision draft

The 2011 Annex 11 remains in force, covering risk management, supplier agreements, validation, audit trails, electronic signatures, data storage, backup, archiving and business continuity. On 7 July 2025 the European Commission and PIC/S published a revision draft (19 pages, 17 chapters) alongside a revised Chapter 4 and a new Annex 22 on AI; the consultation closed on 7 October 2025. As of October 2026 no final text has been published; according to GuideGxP (as of September 2026), finalisation of Annex 11, Annex 22 and Chapter 4 is expected between late 2026 and 2027, with application dates to be set in the final texts and typically a transition period of 6 to 12 months.

The points that matter most for document management are: personal accounts with no shared logins (11.1), multi-factor authentication for remote access to critical systems (11.6), separation of GMP users from administrators (11.10), documented, independent audit trail reviews (12.5, 12.6), complete, searchable electronic copies including the audit trail (12.9), contracts with service providers that define an exit strategy and pre-release testing of new versions (7.5), and read-only archiving with integrity checks (17.1, 17.2). Anyone selecting an eDMS now should assess it against the draft, because inspectors already read its expectations as the current state of the art.

ICH E6(R3): what changes for sponsors and CROs?

ICH E6(R3) reached Step 4 on 6 January 2025; the Principles and Annex 1 have applied in the EU since 23 July 2025. According to ICH, Annex 2 (decentralised and pragmatic elements, real-world data) was adopted on 3 June 2026 and takes effect in the EU on 15 January 2027. New compared with E6(R2) is a dedicated Section 4 on data governance, covering the data lifecycle, metadata and audit trails (4.2) and computerised systems (4.3). Sponsors should keep an inventory of important systems with their validation status and responsibilities, and ensure that validation, audit trails, user management, backup and disaster recovery are proportionately in place. “Essential documents” have become technology-neutral “essential records” (Appendix C), which explicitly accommodates electronic TMFs and eDMSs.

ALCOA+ and data integrity: what does it mean technically?

ALCOA stands for Attributable, Legible, Contemporaneous, Original and Accurate; ALCOA+ adds Complete, Consistent, Enduring and Available. The key guidance documents are the MHRA “GXP Data Integrity Guidance and Definitions” (2018), FDA’s “Data Integrity and Compliance With Drug CGMP” Q&A (2018), PIC/S PI 041-1 (in force since 1 July 2021) and WHO TRS 1033 Annex 4 (2021). The EMA guideline on computerised systems and electronic data in clinical trials (adopted March 2023, in effect since 10 September 2023) uses the term ALCOA++ and adds emphasis on traceability.

Attributable: personal accounts, no shared logins, user ID on every signature and audit trail entry.

Legible: documents and metadata in readable, open formats for the full retention period.

Contemporaneous: system-generated, synchronised timestamps including time zone.

Original: a single master version; copies only as certified copies with metadata.

Accurate: mandatory fields, plausibility checks, controlled templates.

Complete: no deletion without trace; superseded versions are retained.

Consistent: defined statuses and workflows for every document type.

Enduring: backup, restore testing, archiving with integrity verification.

Available: search, export and read-only access for inspectors at any time.

How seriously gaps are treated is shown by the MHRA GCP inspection metrics for 2018/19: 4 of 8 commercial sponsors inspected had at least one critical finding, 7 critical findings in total, 2 of them directly related to the trial master file. No more recent official dataset with a comparable breakdown was available at the time of writing.

What must an audit trail in a GxP eDMS capture?

A GxP audit trail must automatically and immutably record who (user and role) changed what (document, old and new value), when (date, time, time zone) and why. No user may edit or disable it, it must be retained for as long as the record itself, and it must be reviewed periodically on a risk basis. A file-level access log alone does not meet these requirements.

The frameworks differ in emphasis. Part 11 §11.10(e) requires a computer-generated, time-stamped audit trail that does not obscure previous entries; FDA’s 2024 Q&A (Q12) lists date and time, the individual with user ID and role, and old and new values as mandatory components, the reason for change “if applicable”, and recommends a searchable, sortable format. Annex 11 (2011) requires audit trails on a risk basis with regular review. The EMA 2023 guideline expects a secure audit trail stored within the system and procedures for risk-based, trial-specific reviews. The Annex 11 draft is the strictest: systems should automatically prompt for the reason whenever a value changes (12.2), reviews should follow a documented procedure and be carried out by people not involved in the activity (12.5, 12.6), and flat, locked export files are not acceptable (12.9).

Which electronic signature does a GxP eDMS need?

For GxP purposes, a closed system needs a validated electronic signature with unique authentication, timestamp, meaning and a permanent link to the document; an eIDAS qualified electronic signature is not required. Section 4.8 of the EMA 2023 guideline states that the eIDAS Regulation (EU) 910/2014 does not apply to closed systems under its Article 2(2), so the signature functionality has to be proven during validation instead. Only in open systems, where signatories are not known in advance, should signatures at EU sites meet eIDAS.

eIDAS and its amendment eIDAS 2.0 ((EU) 2024/1183) distinguish simple, advanced and qualified signatures; only a qualified signature has the legal effect of a handwritten one. Those tiers answer a question of general law, not a GxP question. The Annex 11 draft requires re-authentication for each signature (13.3), a prompt for meaning (13.5) and a manifestation showing full name, username, role, meaning, date and time (13.6), and refers to trusted services only for open systems or where other legislation requires them (13.2). National rules, for example on electronic informed consent or regulatory submissions, may demand a higher tier and should be checked separately.

How do versioning, approval and retention work in a GxP eDMS?

A GxP eDMS manages the complete document lifecycle: draft, review, approval, effective from a set date, periodic review, superseded and archived. Only the current version may be in use, obsolete versions are retained unchanged, and every approval is electronically signed. Read-and-understood acknowledgements and training records link new SOP versions to the staff they affect.

For GMP the foundation is EU GMP Part I Chapter 4 (Documentation), which requires approved, dated, version-controlled documents and protection against inadvertent use of superseded versions; for US medicines the equivalents are 21 CFR 211.100 and 211.186. For medical devices the Quality Management System Regulation (21 CFR 820) has applied since 2 February 2026, incorporating ISO 13485:2016 and its document control requirements by reference. ICH Q10 anchors document control in the pharmaceutical quality system, and ICH E6(R3) expects sponsors and CROs to run SOP-based processes. Controlled printouts should be labelled as such and traceable.

Retention periods shape the architecture. Under Article 58 of Regulation (EU) 536/2014, the trial master file must be archived for at least 25 years after the end of the trial, on media that keep it complete and legible, and every alteration must be traceable. Directive (EU) 2017/1572 (Art. 9) requires batch documentation to be kept for at least one year after expiry or five years after QP certification, whichever is longer; for investigational products Chapter 4 specifies five years after completion of the last trial. In practice this means exporting to open, long-term formats (such as PDF/A) with metadata and audit trail, documented migrations, and an archive concept that survives a change of supplier.

CSV or CSA: how is an eDMS validated?

An eDMS is now validated on a risk basis: testing effort follows a function’s impact on product quality, patient safety and data integrity rather than a uniform IQ/OQ/PQ template. Traditional computer system validation and computer software assurance are not mutually exclusive; CSA expresses the same risk-based thinking with more emphasis on critical thinking and less documentation for its own sake.

FDA issued the final guidance “Computer Software Assurance for Production and Quality System Software” on 24 September 2025 (draft September 2022). Formally it covers production and quality system software at medical device manufacturers and supersedes Section 6 of the “General Principles of Software Validation”. Its core ideas (intended use, risk classification, unscripted and exploratory testing for lower-risk functions, use of supplier evidence) transfer well to pharma and biotech but are not binding there. Note one tension: the Annex 11 draft expects executed test scripts, with screenshots where relevant (9.4), and traceability of every test to a requirement (9.5).

The methodological framework comes from ISPE GAMP 5 Second Edition (July 2022). It stresses critical thinking, leveraging supplier documentation and supplier audits, handling SaaS and cloud, and agile development models. A configurable eDMS is typically Category 4 software; Category 5 covers bespoke customisations that require considerably more testing.

What does pre-validated mean for an eDMS?

“Pre-validated” means the supplier has tested the standard functions of its system under its own quality management system and provides that evidence as a validation package. It does not mean the system is validated for the customer’s use. Validation for the intended use and release into operation remain the regulated company’s obligation.

A supplier can typically provide requirements and functional specifications, executed test protocols, a traceability matrix, release notes with risk assessment, evidence of its QMS, and support for audits and inspections. The customer retains its own user requirements, a documented risk assessment, review and approval of supplier documentation, PQ or user acceptance testing for its own processes and configuration, SOPs, training and the decision to release the system for use.

The frameworks leave no doubt. The Annex 11 draft states that a regulated user relying on a supplier’s qualification or operation remains fully responsible (7.1) and must carefully review and authorise supplier documentation (9.9); Section 3 of the 2011 Annex 11 already requires supplied documentation to be checked against user requirements. Annex 2 (A2.1) of the EMA 2023 guideline allows reliance on supplier validation documentation once it has been assessed as adequate, while ultimate responsibility stays with the responsible sponsor or investigator, and inspectors are given access to the full documentation including the company’s own supplier assessment. Under that guideline, providers unwilling to give access to documentation or to support inspections should not be used. FDA’s 2024 Q&A (Q7) likewise expects validation documentation, including the IT provider’s, to be available at inspection.

To be fair to both sides, supplier documentation largely suffices for unmodified standard functions from a supplier with an audited QMS. Findings arise when companies adopt a certificate or validation package without review, define no requirements of their own, leave configurations untested or accept SaaS updates without assessment. For continuous releases, contracts should provide advance notice, release notes with impact analysis and a test environment before production roll-out, as described in Annex 11 draft 7.5 and the EMA guideline (6.7); internally, each release belongs in change control with a documented assessment.

What is the difference between an eDMS, an eQMS and an eTMF?

An eDMS manages controlled documents such as SOPs, policies and forms throughout their lifecycle; an eQMS adds quality processes such as CAPA, deviations, change control, audits, training and supplier management; an eTMF manages the essential records of a clinical trial. The systems overlap on SOPs, training records and document change control. Which one is needed depends on the scope of a company’s own GxP activities, not on headcount alone.

An eTMF usually structures trial documents according to the TMF Reference Model, whose version 3.3.1 (August 2023) has industry adoption of over 90% according to Just In Time GCP, a member of the Steering Committee, and according to Kivo underpins most eTMF systems in production; at the CDISC + TMF US Interchange in Nashville in October 2025, the TMF project team announced that the planned version 4 has evolved into Version 1 of a formal, digital, machine-readable CDISC TMF Standard, which Just In Time GCP expects to be published in 2027. Content expectations are set out in the EMA “Guideline on the content, management and archiving of the clinical trial master file” (EMA/INS/GCP/856758/2018), including completeness, inspection readiness and direct read-only access for inspectors.

eDMS, eQMS, eTMF and generic cloud storage compared (as of October 2026)

System type

Core function

Typical need

Limitation

eDMS

Lifecycle of controlled documents, signatures, audit trail, read acknowledgement

Preclinical, early phases, small SOP base

No structured CAPA, deviation or supplier management

eQMS

eDMS plus CAPA, deviations, change control, audits, training, suppliers

Own GMP activities, Phase II/III, inspections

Greater implementation and validation effort

eTMF

Essential records per trial, Reference Model structure, completeness tracking

Sponsor holds its own TMF, several trials

Not designed for company-wide SOP control

Generic cloud storage

Filing, file versioning, retention policies, activity logs

Non-GxP documents, collaboration

GxP functions only through customisation and own validation

Point solutions can be introduced faster and chosen selectively, but they create interfaces, duplicate user administration and multiple validations. Integrated platforms cut out hand-offs, for instance when an SOP revision automatically triggers training tasks, but tie a company more closely to one supplier. For small teams with growing needs, the analysis therefore favours an integrated, pre-validated platform that starts modularly with the eDMS and adds eQMS processes later, provided data export and an exit strategy are secured contractually. A detailed supplier comparison is available here: eQMS software comparison and eTMF software comparison. The fundamentals of a GxP eDMS are covered separately.

Is SharePoint enough for GxP document management?

SharePoint and other generic cloud storage services are not a GxP eDMS out of the box; they provide building blocks such as versioning, retention policies and activity logs, but no validated GxP workflows. GxP compliance comes only through configuration, extensions, validation of the organisation’s own configuration and ongoing maintenance. For very small organisations that can be defensible; as document volumes grow and inspections approach, it becomes a risk.

On Microsoft Learn, Microsoft itself states that “there is no GxP or FDA 21 CFR Part 11 certification for cloud service providers”, and its GxP documentation, based on a GxP qualification review carried out by Montrium, describes a shared responsibility model in which the customer takes on validation and governance (supplier statement). Typical gaps in generic systems are: no Part 11-compliant signature manifestation with meaning and re-authentication; file-level activity logs instead of an audit trail with old and new values and a reason for change; no lifecycle statuses with effective dates and periodic review; administrators able to delete or alter content; and platform updates on the vendor’s timetable, outside the company’s own change control.

A validated SharePoint solution can be defensible where only a few dozen SOPs are managed, internal IT or consultancy capacity exists for configuration, validation and release assessment, and no inspection is imminent. It becomes a risk when undocumented customisation accumulates, key people leave, or signature and audit trail requirements are covered only by procedures. For a fuller comparison, see SharePoint vs. eDMS.

Why does EU hosting matter for European biotechs?

EU hosting reduces the legal risk of third-country transfers under the GDPR and of access by foreign authorities, but it is not an explicit GxP requirement. The EMA 2023 guideline does not prescribe a data location; it does require the storage location and control to be described, data jurisdiction questions to be settled contractually before use, and inspectors to have direct read-only access at any time. For trial documents containing health data, an EU location is therefore usually the lower-risk choice.

Health data is a special category under Article 9 GDPR, and pseudonymised trial data remains personal data. Under Articles 44 to 49 GDPR, transfers to third countries require an adequacy decision, appropriate safeguards or a derogation. In Schrems II (C-311/18) in 2020 the CJEU struck down the Privacy Shield; the adequacy decision for the EU-US Data Privacy Framework has applied since July 2023. On 3 September 2025 the General Court dismissed the annulment action in Latombe (T-553/23); the appeal, C-703/25 P, was lodged on 31 October 2025 and, according to the Court of Justice, is pending as of October 2026. Separately, the US CLOUD Act of 2018 can oblige US providers to hand over data held in EU data centres.

Other frameworks also bear on the choice. The Data Act ((EU) 2023/2854) has applied since 12 September 2025, gives customers switching rights and bans switching charges, including egress fees, from 12 January 2027. The NIS2 Directive ((EU) 2022/2555) covers cloud providers and, in Annex I (health sector), explicitly lists entities carrying out research and development of medicinal products and manufacturers of pharmaceutical products; under Article 3, medium-sized companies with 50 or more staff or more than €10 million turnover or balance sheet total count as important entities, and larger ones as essential entities. The BSI C5 attestation and ISO/IEC 27001 are robust evidence of a supplier’s security. Background on EU-hosted document management is covered separately.

Which selection criteria should small organisations apply to a GxP eDMS?

Small organisations should assess an eDMS against seven areas: regulation, functionality, validation, hosting, operations, exit strategy and inspection readiness. The decisive question is how much in-house effort a system requires, because small teams cannot run validation and maintenance as a side task.

Regulation: coverage of Part 11, Annex 11 (including the revision draft), ICH E6(R3) and the EMA 2023 guideline, evidenced rather than merely claimed.

Audit trail: automatic, non-editable, with role, old and new value, reason and time zone; searchable and exportable.

Signatures: re-authentication, meaning, manifestation on screen and in print, invalidation on later change.

Lifecycle: configurable workflows, effective dates, periodic review, read acknowledgement and training records.

Access: personal accounts, roles, MFA, separation of user and administrator rights, documented access reviews.

Validation: validation package with test protocols and traceability matrix, auditable supplier QMS, templates for URS and UAT.

Release management: advance notice, release notes with risk assessment, test environment before roll-out.

Hosting: EU data centres, no unresolved third-country access, data processing agreement, evidence such as C5 or ISO/IEC 27001.

Exit: contractually guaranteed full export of documents, metadata and audit trail in open formats.

Inspection readiness: read-only access for inspectors, inspection support, data-flow diagram and system inventory.

Effort: realistic implementation time, internal roles, maintenance effort per release.

Scalability: adding eQMS or eTMF functions later without migration.

Which system suits which situation?

Preclinical biotech with 10 staff: A pre-validated eDMS for SOPs, policies and training records is usually sufficient. What matters is a clean lifecycle from day one, so that no legacy data has to be migrated later.

Phase I sponsor with outsourced clinical operations: An eDMS for in-house SOPs is generally enough when the CRO keeps the TMF in its eTMF. The sponsor nonetheless remains responsible for the TMF and needs contractually secured read access and a handover at the end of the trial.

Phase II/III sponsor ahead of its first inspection: At this point an eQMS with deviations, CAPA, change control and supplier qualification becomes necessary, often supplemented by an in-house eTMF. An integrated platform reduces interfaces and the validation burden on a small quality team.

Small CRO serving several sponsors: It needs an eQMS for its own processes and a multi-client structure that keeps sponsor documents cleanly separated. Sponsors will audit the CRO’s systems, so validation evidence must be ready to show at any time.

Frequently asked questions about GxP eDMS

What is a GxP eDMS?

A GxP eDMS is a validated electronic document management system that manages controlled documents in line with GMP, GCP or GLP requirements. It provides an audit trail, electronic signatures, versioning and approval workflows that meet 21 CFR Part 11 and EU GMP Annex 11.

Is a qualified electronic signature mandatory for GMP or GCP in the EU?

Not as a general rule. According to the EMA 2023 guideline, eIDAS does not apply to closed systems, although the signature must be validated; national rules, for example on electronic consent, may require a higher tier.

Does the new Annex 11 already apply?

No. As of October 2026 the 2011 version remains in force and the July 2025 draft is not yet final. Inspectors are, however, already guided by its expectations.

Does CSA replace computer system validation?

CSA is a risk-based form of validation that FDA finalised in September 2025 for medical device software. It is not binding for pharma, but its principles are consistent with GAMP 5.

Can a supplier validate an eDMS for me?

A supplier can test standard functions and provide documentation. Validation for your intended use, review of that documentation and release for use remain with the regulated company.

How long must trial documents be retained in the EU?

Under Article 58 of Regulation (EU) 536/2014, the trial master file must be archived for at least 25 years after the end of the trial, complete, legible and with every change traceable.

Is Microsoft 365 sufficient for GxP documents?

Only with substantial configuration, extensions and your own validation. Microsoft itself describes a shared responsibility model in which the customer is responsible for GxP compliance.

Does a GxP eDMS have to be hosted in the EU?

GxP rules do not prescribe a location, but they do require settled jurisdiction and inspector access. Because of GDPR risks around third-country transfers, EU hosting is usually the safer choice for trials involving health data.

Sources

https://fda.gov/media/166215/download

FDA Releases Final Guidance Q&A on Electronic Systems, Records, and Signatures — Clinical Pathways

FDA Electronic Systems, Records &
